Required Documents For Background Investigations


It will be necessary to submit your completed Personal Information and History Packet and additional required documents upon the passing of the applicant’s written test and VPD Review Board. Your background process will not begin until all of these forms have been turned in.

This list is provided as a guide for applicants to gather the documents required for the position for which they are applying.

This document will need to be completed in its entirety.  Pay close attention to the instructions given in each section.

The following items will be required from each applicant as applicable:

  • Authority to Release Information - TCOLE form (Police Officer Applicants Only) (Notarized) (Included in the Personal Information and History Packet)
  • Birth Certificate
  • City of Victoria Personal History Statement - (Notarized) (Included in the Personal Information and History Packet)
  • College Diploma
  • College Transcript (Certified / Official)
  • Confidential Information Agreement Form - (Notarized) (Included in the Personal Information and History Packet)
  • Copy of Driver’s License
  • Copy of Social Security Card
  • High School Diploma / GED Certificate
  • High School Transcript (Certified / Official)
  • Military Discharge / DD214
  • Police Academy Diploma
  • TCOLE Exam Results (if known)
  • TCOLE Form F-5R (if currently or previously licensed)
  • TCOLE License
  • Training Certificates (if applicable)

Other items may be required by your background investigator as the investigation progresses.
